Closed-loop administration of propofol for the control of hypnosis in anesthesia has evidenced an outperformance when comparing it with manual administration in terms of drug consumption and post-operative recovery of patients. Unlike other systems, the success of this strategy lies on the availability of a feedback variable capable of quantifying the current hypnotic state of the patient. However, the appearance of anomalies during the anesthetic process may result in inaccurate actions of the automatic controller. These anomalies may come from the (...) monitors, the syringe pumps, the actions of the surgeon or even from alterations in patients. This could produce adverse side effects that can affect the patient postoperative and reduce the safety of the patient in the operating room. Then, the use of anomaly detection techniques plays a significant role to avoid this undesirable situations. This work assesses different one-class intelligent techniques to detect anomalies in patients undergoing general anesthesia. Due to the difficulty of obtaining real data from anomaly situations, artificial outliers are generated to check the performance of each classifier. The final model presents successful performance. IntroductionRheumatologists are the primary healthcare professionals responsible for patients with rheumatic diseases and should acquire medical ethical competencies, such as the informed consent process. The objective clinical structured examination is a valuable tool for assessing clinical competencies. We report the performance of 90 rheumatologist trainees participating in a station designed to evaluate the ICP during the 2018 and 2019 national accreditations.MethodsThe station was validated and represented a medical encounter in which the rheumatologist informed a patient with systemic lupus erythematosus with (...) clinically active nephritis about renal biopsy. A trained patient–actor and an evaluator were instructed to assess ICP skills in obtaining formal informed consent, delivering bad news and overall communication with patients. The evaluator used a tailored checklist and form.ResultsCandidate performance varied with ICP content and was superior for potential benefit information but significantly reduced for potential complications and biopsy description. Only 17.8% of the candidates mentioned the legal perspective of ICP. Death was omitted by the majority of the candidates ; after the patient–actor challenged candidates, only 57.1% of them gave a clear and positive answer. Evaluators frequently rated candidate communications skills as superior, but ≥1 negative aspect was identified in 69% of the candidates.ConclusionsEthical competencies are mandatory for professional rheumatologists. It seems necessary to include an ethics competency framework in the curriculum throughout the rheumatology residency. (shrink)

Given the restrictions of space (a limitation they too had to contend with), we can only respond to a few aspects of their interesting remarks. (...) We shall reply of course on the basis of our own approach—an approach that sets out to integrate culture, behavior, and experience, and that might be termed contextual phenomenology, phenomenological behaviorism, or perhaps most accurately (albeit more awkwardly), the phenomenological–behavioral perspective.In her commentary, Ibáñez-Guerra (2008), referring to the target articles “Phenomenology and behaviorism” (Pérez-Álvarez, Sass, and Cangas 2008) and “The role of superstition in psychopathology” (García-Montes et al. 2008), wonders whether our maintaining of the spirit of Skinner, as well as our recourse to phenomenology, might not themselves represent forms of superstitious behavior. She points out that it is not always a good idea to put new wine in old barrels. For her, the new wine and new barrels would be represented by social constructivism, of which she is reminded by some aspects of our work. But in making her own case, Ibáñez-Guerra (2008) reveals certain common prejudices concerning behaviorism, prejudices that have apparently persisted even after reading our article. Thus, for example, she does not see operant behavior as incorporating intentionality (what, in the target article, is presented in terms of final causality), because she assumes that intentionality would necessarily imply a mentalist and dualist notion involving a supposed internal mental entity that causes behavior. Skinner (1974) himself noted and rejected this very assumption in a passage that clearly distinguishes his own approach from what he calls a “stimulus-response formula”: “Possibly no charge,” he wrote,is more often leveled against behaviorism or a science of behavior than that it cannot deal with purpose or intention. A stimulus-response formula has no answer, but operant behavior is the very field of purpose and [End Page 281] intention. By its nature it is directed toward the future: a person acts in order that something will happen, and the order is temporal.(pp. 55–6)The place Skinner gives to intentionality and purpose is elaborated in our target article.Furthermore, Ibáñez-Guerra (2008) assumes that, on methodological grounds, Skinner rejects all that is not observable, or at least that cannot be operationalized. This, however, is simply incorrect, at least as regards Skinner’s radical behaviorism. As the reader may recall, radical (as opposed to methodological) behaviorism is neither operationalist nor does it reject private or subjective events because of their unobservability. Indeed, Skinner specifically acknowledges that such events are in fact observable—at least for one person: the subject him- or herself. The question for Skinner is to understand how the part of the world that is only observable and experiential for oneself is constructed or formed. In the end, it turns out that radical behaviorism is more radical, in the Aristotelian ontological sense (see Pérez-Alvarez, Sass, and García-Montes 2008), than post-modern constructionism. Indeed, as we remarked, radical behaviorism can still surprise us today. Similarly, phenomenology is starting to be missed in the clinical context (Andreasen 2007). We believe that radical behaviorism and phenomenology, mutually readjusted, are in fact excellent wines for the times we live in—if only we knew how to appreciate them.In their commentary on “Phenomenology and behaviorism: A mutual readjustment,” Fletcher and Hayes (2008) argue that the philosophy and theory of Acceptance and Commitment Therapy (ACT)/Relational Frame Theory (RFT) already contain the salutary aspects of phenomenology that we highlight in our target article. This work begins by proposing the need for exploring the mode of being of mental disorders. It is a philosophical study in an Aristotelian perspective, with special emphasis on the anthropological–cultural dimension. It is difficult for such an inquiry to be carried out from within psychiatry or clinical psychology, committed as these fields are to their own logic and practical conditions. The issues are, in any case, more ontological than strictly clinical in nature. We therefore turn to Aristotle, and specifically (...) his doctrine of the four “causes,” to flesh out the social and cultural dimensions of mental disorders. In accordance with the present analysis, the material cause of disorders would be found in the contingencies of life; the formal cause would pertain primarily to the way clinical conditions themselves can serve as models of ‘being ill’ in our society; the efficient cause would correspond to the patients themselves, understood as active (albeit less than fully conscious) agents as well as to the pharmaceutical industry and the mass media; the final cause would be found in different adaptive functions served by the disorder. We conclude that the “mode of being” of most (if not all) mental disorders—in particular, their status as mental disorders—can often have more to do with cultural forms than with biological factors. This article considers the relationship between phenomenology and behaviorism in a new perspective. First, we present the phenomenological approach of the Spanish philosopher Ortega y Gasset (1883–1953). Ortega’s perspective involves a transformation of classical phenomenology in a direction that emphasizes ‘life as action’ and ‘historical reason’ as a form of explanation. These aspects of Ortega’s approach are of interest to contemporary phenomenology, and enable phenomenology’s relationship with behaviorism to be reconsidered. Second, we present Skinner’s radical behaviorism, the variant of behaviorism (...) most relevant to phenomenology. Of particular importance here is radical behaviorism’s emphasis on final causality and its approach to ‘private events’ in terms of the interpersonal functions served by these events. Third, we propose a mutual correction, both of behaviorism by phenomenology and of phenomenology by behaviorism, in which behaviorism’s notion of the environment may better be conceptualized through the phenomenological notion of the ‘lived-world,’ and phenomenology could adopt radical behaviorism’s dialectical and constructivist perspective. As a result of recent corporate scandals, several rules have focused on the role played by Boards of Directors on the planning and monitoring of corporate codes of ethics. In theory, outside directors are in a better position than insiders to protect and further the interests of all stakeholders because of their experience and their sense of moral and legal obligations. Female directors also tend to be more sensitive to ethics according to several past studies which explain this affirmation by (...) early gender socialization, the fact that women are thought to place a greater emphasis on harmonious relations and the fact that men and women use different ethical frameworks in their judgments. The goal of this paper is to determine the influence of these characteristics of the Board in terms of promoting and hindering the creation of a code of ethics. Our findings show that a greater number of female directors does not necessarily lead to more ethical companies. Moreover, within Europe as a continent, board ownership leads to an entrenchment of upper-level management, generating a divergence between the ethical interests of owners and managers. In light of this situation, the presence of independent directors is necessary to reduce such conflicts. (shrink)

The analysis of so-called ?strategic intentional fouls? (SIF) as well as the discussion of their validity in the normative systems of sports have a long track record. These fouls can be characterised as rule violations committed in order to be detected and which accept the corresponding sanction. However, there is an additional goal of obtaining an advantage or subsequent benefit in the competition. In fact, this practice is not infrequent and it is even occasionally accepted by the players themselves, referees, (...) judges, sports authorities and spectators. In this work I have analysed the internal structure of SIFs (the features of an axiological gap and a ?special intention?) with a view to providing a deeper understanding and stressing the differences with closely related concepts: cheating, and especially fraus legis or what in Anglo-Saxon culture is called ?spoiling the game?. Finally, I try to show some difficulties in distinguishing SIF from gamesmanship. (shrink)

A su vez, trata de mostrar las diversas analogías entre las propiedades ambos conceptos (omnipotencia, unidad, ilimitabilidad, indivisibilidad), así como: 1° los problemas explicativos de la relación Dios-mundo y Estado-Derecho ; 2° la analogía entre el milagro y el poder constituyente originario ; 3° la afinidad entre el dogma de la infalibilidad papal y la cosa juzgada. Este trabajo presenta los aspectos más importantes de las nuevas Directrices de Bautismo de la Archidiócesis de Sevilla situándolas en el contexto histórico teológico en el que maduran y destacando los principales criterios pastorales que utilizan.

En este trabajo se presenta el documento de la Conferencia Episcopal Española sobre la Iniciación Cristiana, publicado en noviembre de 1998. Se destacan los aspectos más relevantes desde el punto de vista pastoral y se plantean algunas cuestiones aún por resolver.

La presente nota bibliográfica recopila la ingente obra de José Aldazábal, uno de los liturgistas españoles más fecundos en publicaciones de carácter sobre todo divulgativo, que, durante más de cuarenta años y formando parte del Centro de Pastoral Litúrgica de Barcelona, ha acompañado la pastoral litúrgica especialmente en España, pero también en los paísesiberoamericanos de lengua española. Interesará a los investigadores de la historia de la renovación litúrgica y a los estudiosos de la pastoral.

Esta nota, que resume una conferencia pronunciada en octubre de 2011 en las Jornadas Nacionales de Liturgia de Barcelona, analiza y estudia la aportación de Sacramentum Caritatis al importante tema de la participación activa de los fieles en la liturgia. El autor concluye, entre otras cosas, que la sutil distinción entre condiciones personales para la participación y la participación misma enriquece el concepto de actuosa participatio acentuando sus dimensiones más hondas desde el punto de vista de la antropología sacramental.

Recientemente se han ido introduciendo progresivamente en el discurso litúrgico dos cuestiones que se relacionan estrechamente con la noción teológica de la Liturgia y con la Pastoral litúrgica en acto: la cuestión del arte de celebrar y la del arte de participar. En esta nota recojo algunas consideraciones sobre ambas cuestiones partiendo de la noción de celebración que privilegia la Constitución de Liturgia.

A partir de la distinción entre culto genéricamente religioso y culto cristiano se reflexiona sobre la novedad de la Liturgia cristiana, en cuya naturaleza pesa más la dimensión descendente, o sea, su cualidad de ser actualización, aquí y hoy, del Misterio de Cristo que la dimensión cultual o ascendente, o sea la glorificación de Dios.

The life of Saint Teresa of Jesus, the most famous mystic of sixteenth-century Spain, was characterized by recurrent visions and states of ecstasy. In this paper, we examine social components related to Teresa’s personal crises and the historical conditions of her times, factors that must be taken into account to understand these unusual forms of experience and behavior. Many of these factors (e.g., increasing individualism and reflexivity) are precursors of the condition of modern times. Indeed, certain parallels can be observed (...) between Saint Teresa and certain present-day psychopathological disorders. The analogy should not, however, be carried too far. Religion played a particularly crucial role in Teresa’s cultural context; as a result, it would be misleading to view her mystical experiences as resulting from a mental disorder. (shrink)

The fault prediction and abductive fault diagnosis of three-phase induction motors are of great importance for improving their working safety, reliability, and economy; however, it is difficult to succeed in solving these issues. This paper proposes a fault analysis method of motors based on modified fuzzy reasoning spiking neural P systems with real numbers for fault prediction and abductive fault diagnosis. To achieve this goal, fault fuzzy production rules of three-phase induction motors are first proposed. Then, the rMFRSNPS is presented (...) to model the rules, which provides an intuitive way for modelling the motors. Moreover, to realize the parallel data computing and information reasoning in the fault prediction and diagnosis process, three reasoning algorithms for the rMFRSNPS are proposed: the pulse value reasoning algorithm, the forward fault prediction reasoning algorithm, and the backward abductive fault diagnosis reasoning algorithm. Finally, some case studies are given, in order to verify the feasibility and effectiveness of the proposed method. (shrink)

Resumen. La presente investigación ofrece la oportunidad de pensar y dialogar sobre la estrategia, los objetivos, las personas y el futuro de la gestión municipal del H. Ayuntamiento de Navolato desde una perspectiva del conocimiento. Se trata pues de averiguar qué características deben cumplir esos recursos para adquirir un carácter estratégico. La información se obtuvo mediante una encuesta semiestructurada, aplicada a 181 individuos. Los datos recabados fueron objeto, en un principio, de dos tipos de tabulaciones. En algunos casos sólo se (...) efectúo una tabulación simple, que consistió en el conteo de una sola variable; en otros casos la tabulación fue cruzada contando dos o más variables de forma simultánea. Para el análisis de los datos se utilizó el programa de tratamiento de datos estadísticos SPSS versión 12 para Windows, que facilitó el tratamiento inferencial de los datos extraídos de la muestra seleccionada. El análisis de los resultados muestra que el Ayuntamiento presenta perfiles favorables para una gestión del conocimiento, óptima y eficaz, al alcanzar una puntuación total de 115, contando con una fortaleza estructural interna. Se concluye que las administraciones públicas no son ajenas a la sociedad del conocimiento, se están acomodando a estas nuevas exigencias, mejorando sus procesos para satisfacer las nuevas necesidades de los ciudadanos. Redefiniendo la función pública y el servicio público, a través de la búsqueda de mecanismos para conocer y medir el valor de la gestión del conocimiento, y así poder llevar a cabo una gestión eficiente y eficaz de estos recursos.Palabras Claves. This research provides an opportunity to think and discuss the strategy, goals, people and future of municipal management of Navolato City knowledge perspective. It is about finding out what characteristics should meet those resources to acquire a strategic nature. The information was obtained through a semistructured survey applied to 181 individuals. The data collected were subjected, in principle, two types of tabs. In some cases only made a simple tabulation, which consisted of a single variable count, in other cases the cross tabulation was counting two or more variables simultaneously. For data analysis software was used for statistical data processing SPSS version 12 for Windows, which facilitated inferential treatment of the extracted data from the selected sample. The analysis of the results shows that the City has profiles favorable for knowledge management, optimal and efficient, to achieve a total score of 115, relying on internal structural strength. We conclude that public administrations are not foreign to the knowledge society, are arranging to these new demands by improving their processes to satisfy the new needs of citizens. Redefining the civil and public service, through the search for mechanisms to analyze and measure the value of knowledge management, and therefore be able to carry out a efficient and effective management of these resources.Keywords. RESUMEN El objetivo del presente estudio está dirigido a exponer particularidades éticas del trabajo anatomopatológico a la luz de la realidad histórico social. Se emplean métodos teóricos y empíricos y a partir del análisis documental se opera con referentes en los enfoques contemporáneos. El estudio de aspectos generales referidos a la moral y a la ética permitió acceder a definiciones, bases legales y evolución histórica, a la vez que se profundiza en su repercusión en el sector salud y se desarrollan (...) las peculiaridades del trabajo anatomopatológico desde sus manifestaciones en la actividad asistencial, docente e investigativa. Se concluye que los principios éticos generales de la conducta profesional y médica son aplicables al trabajo anatomopatológico en todas sus esferas; al igual que los de la bioética. ABSTRACT The objective of the present study is directed to exhibit ethical peculiarities of the anatomical-pathological work in view of the historical-social reality. Theoretical and empirical methods are used and from the documentary analysis it occurs with modality in the contemporary approaches. The study of general aspects referred to the morality and to the ethics allowed to gain access to definitions, legal bases and historical evolution, simultaneously that deepens in its aftereffect in the sector health and the peculiarities of the anatomical-pathological work develop from its declarations in the welfare, teaching and investigative activity. One concludes that the general ethical beginning of the professional and medical conduct is applicable to the anatomical-pathological work in all its spheres; as those of the bioethics. (shrink)
